To dispute report findings, request a copy of the completed report, or request the removal of adverse information in cases of human trafficking, please complete the below form:

Requests to block adverse information resulting from human trafficking must include the following:
- Proof of your identity (e.g., scanned driver’s license or passport)
- Trafficking documentation, including at least one of the following:
- A determination of victim status from a federal, state, or tribal government entity
- A determination of victim status from a non-governmental organization or member of a human trafficking task force authorized by the government to make such a determination
- Court documents showing your victim status
- A statement signed by you or your representative attesting that you are a victim of trafficking. The statement or an accompanying document must also be signed by a government entity, a non-governmental organization, a human trafficking task force, or a court
- Identification of the adverse information that resulted from human trafficking that you wish to block
AUTHORIZATION: By providing the above information, you are certifying that you are the candidate/applicant whose information you are disputing or requesting, that the information is current and complete, and that you are aware that you can contact customer service at 1(888) 462-2251, Option 1 or service@trustHR.com if you have any questions about requesting a copy of your report. In instances of disputes or requests for the removal of adverse information in cases of human trafficking, we will provide you with a separate link to upload any additional documents that may support your dispute or request for removal.
